Registered Nurses advocate for patient’s health, coordinate healthcare services and help patients understand their health conditions.
In addition to shared decisions with patients and providers, Registered Nurses empower patients through education and link patients to the right resources at the right time.
They provide customized clinical education related to:
- acute and chronic conditions, such as diabetes, high blood pressure, heart disease, asthma and other illnesses
- medication management,
- nutrition,
- physical activity,
- smoking cessation and prevention.
RN Care Coordinators meet patients where they are at, by offering patients individualized assistance. Patients see FRRH RN Care Coordinators as their personal liaison on their personalized care team. The RN Care Coordinators enhance patients’ quality of life by networking needed resources and supports to navigate the complex healthcare system.
